All students who are under the age of 18 as of the first day of class must have their parent or guardian sign a waiver form before class begins. Your class registration is NOT COMPLETE until you send us your waiver.

Download the waiver here & have your parent or guardian fill in the information requested. 

Return your completed waiver BEFORE THE FIRST DAY OF CLASS
